Applying for Assistance - 2010-2011 School Year

Request Information and Forms

  • For New Families: request an admissions application packet and submit completed forms to Marta Miskolczy, Director of Admissions.  Tuition assistance information is included in the admissions packet.

  • For Returning Families: By February 11, 2010, complete and submit a letter of intent for ongoing tuition assistance for the coming school year.

Complete Parents’ Financial Statement

Santa Fe Prep requires that any family requesting tuition assistance file a Parents’ Financial Statement (PFS), available in November for the following school year, through School and Student Services (SSS).  The PFS becomes the official application for assistance and can be completed and submitted on line.  Questions about completing the PFS should be directed to SSS at 800-344-8328.  Please note that the PFS is an application for tuition assistance and is NOT an application for admission to Prep.

If filing electronically is unavailable for your family, you may request the use of a Prep computer or you may fill out a paper version of the document.  Please contact , Financial Aid Coordinator, to request the paper version or set up an appointment to use a School computer.  Please be aware that the processing time for mail-in applications is much longer than it is for applications submitted electronically. Meeting all stated deadlines is critical.

Both custodial and non-custodial parents must complete the PFS.  Santa Fe Prep requires financial information from each step parent, guardian or other adult wage earner living in the home.

Submit Parents’ Financial Statement

The PFS must be post-marked or submitted on-line on or before February 22, 2010. If submitting the PFS on-line, the processing fee is $35 and a credit card is necessary.  If you are submitting the paper version, please mail it to SSS by NAIS at the same address listed below for submitting tax documents. There is a $42 fee that must be included with your PFS.  We do have some fee waivers available to new families who qualify.  Santa Fe Prep’s school code is 7138.  Use this number in PFS item 5(A). 

Submit Tax Information

On or before February 22, 2010, mail your IRS and W-2 forms to SSS by NAIS, Application Processing Center, PO Box 449, Randolph, MA 02368-0449.  For overnight mail, use SSS by NAIS, Application Processing Center, 437 Turnpike Street, Canton, MA 02021. Include:

  • A signed copy of your 2009 IRS Tax Form 1040
  • All accompanying schedules, such as form W-2s, form 1099, and any other attachments
  • Any business, corporate, or sole proprietorship forms, such as 1020S, Schedule C, K-1,etc.
  • A Business/Farm Statement from SSS, to be completed by owners of businesses or farms (included with the tuition assistance information or available on line.
  • Signed IRS form 4506-T (included with the tuition assistance information or available on line Do not date the form 4506-T!

Notification

If paperwork is submitted on or before February 22, 2010, and the student is accepted to Prep, families will be notified of any tuition assistance awards by April 2nd, with the acceptance letter.  For returning Prep students, families will receive notification of any awards in April.
